Current Fees: $150/clinical hour (50-55 minutes)
Late Cancellation (anything within one full business day): $40
No-show: $60

Please park away from the mailbox, so as not to interfere with mail delivery.

NOTE: Behind the brick mailbox, on the south end of the property (to the RIGHT side, when looking at the front door), you will see a sidewalk that leads to BASEMENT stairs. Go down the stairs, ring the bell and walk into the waiting room. You will probably not hear the doorbell; it rings upstairs.

Payment
Payment is expected at the time of service. Payment may be made with cash, check, or credit card (MC, VISA, or DISCOVER).

If a bishop or other third party is paying for your counseling, billing arrangements can be made by giving that information to Faith (801.656.7037).

Release of Confidential Information
If a bishop or other third party is paying for your counseling, you will need to sign a Release form which authorizes us to share billing information.

Insurance
Anderson Counseling does not bill insurance and is not a provider for any insurance companies. However, some insurance companies will still cover a portion of the costs. Call your insurance company to find out if they will do so. If so, you will need to pay for services and Anderson Counseling will provide you with a superbill receipt with the information necessary to file a claim with your insurance company for reimbursement.

Appointments
Earliest appointment begins at 9a, latest appointment begins at 5p.

When contacting Faith (801.656.7037), be sure to leave a message as she is rarely able to pick up the phone immediately.
